Confirmation Order This is the contract between yourself and Movements International.
Client Contact FormSupply full contact details for both origin & destination.
Alcohol List A list of all alcohol in your shipment is required for Customs Clearance.

Insurance:
Financial Services Guide (FSG) &Product Disclosure Statement (PDS)These FSG explains the insurance services we provide as an authorised representative of a licensed broker the PDS contains information about the insurance.
Insurance Policy WordingSummarises the Insurance Policy and supplies definitions of the wording.
Insurance Declaration This form is the valuation list for all items that you wish to insure. You only need to complete this form if Movements International have not physically inspected the items to be moved.
Claim Form Unfortunately accidents sometimes happen, should you need to lodge a claim please complete the attached forms and post (address on forms), email to MIM at sydney@mim.net.au or fax +61 2 9725 3727
OTHER DOCUMENTS
Hazardous Goods Declaration A signed declaration confirming that there are NO Hazardous Goods in your consignment may be required.
Credit Card Authority Form Wish to pay by Credit Card? Complete and fax to MIM office.
Pre-Removal ChecklistA list of things to do prior to your removal.
Crate Request Form Require crates for your removal? Complete and fax to MIM office.
